Preparing for Your

FIRST VISIT

1. Upon scheduling, you will either receive links to our electronic system via text/email or you will be mailed paper forms. We ask that these be completed prior to arrival.

2. All new patients will receive a comprehensive evaluation at this appointment unless it is an emergency situation. Any needed diagnostics, such as x-rays will be taken as well.

3. For patients under the age of 18, we ask to have a parent or guardian attend the initial visit.

4. Please let us know prior to your appointment if you have any special medical needs or have been told you need to take an antibiotic prior to your appointment.

5. Bring a list of current prescription and over-the-counter medications
